Roles & Responsibilities

• Provide compassionate bedside care and monitor patient conditions
• Administer medications and treatments as prescribed
• Collaborate with RNs and physicians to implement and adjust care plans
• Document patient vitals, symptoms, and progress accurately
• Assist with daily living activities and ensure patient comfort
• Educate patients and their families on care plans and health management

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification (American Heart Association preferred)
• Previous long-term care or skilled nursing experience preferred
• Strong assessment and communication skills
• Ability to work flexible per diem shifts

Knowledge & Skills

• Knowledge of nursing care plans, medication administration, and patient assessment
• Proficiency with electronic health records (EHR) documentation
• Strong critical thinking and problem-solving abilities
• Excellent interpersonal and patient education skills
• Compassionate, patient-centered approach to care
