Roles & Responsibilities

• Provide compassionate direct patient care, including hygiene, mobility, and comfort measures
• Administer medications and treatments as ordered, monitoring patients for response
• Take and record vital signs, reporting changes in patient condition to the care team
• Collaborate with registered nurses, physicians, and other healthcare professionals to deliver quality care
• Maintain accurate, timely documentation of patient care and observations
• Educate patients and families on health conditions and care plans, empowering them in their care journey

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• BLS/CPR certification (American Heart Association or equivalent)
• Previous LPN experience in a clinical or long-term care setting is preferred but not required
• Strong clinical assessment and critical thinking abilities
• Excellent communication and interpersonal skills, with a caring and professional demeanor

Knowledge & Skills

• Medication administration and dosage calculation
• Patient assessment and vital sign monitoring
• Electronic health records (EHR) documentation
• Infection control and safety protocols
