Roles & Responsibilities

• Provide direct, compassionate nursing care to patients in a skilled nursing facility
• Administer medications and treatments as prescribed, monitoring for effectiveness and side effects
• Assess patient conditions, document vitals, and report any changes to the healthcare team
• Collaborate with nurses, physicians, and other professionals to develop and adjust care plans
• Educate patients and their families on health management and post-care instructions

Qualifications

• Active LPN license in the state of Pennsylvania
• Current BLS/CPR certification
• Strong clinical assessment and critical thinking skills
• Ability to work independently while being a team player
• Compassionate, patient-centered approach to care

Knowledge & Skills

• Solid understanding of nursing fundamentals and medication administration
• Excellent communication and interpersonal skills
• Proficiency in documenting care and using electronic health records
• Ability to adapt quickly to changing patient needs
• Knowledge of infection control practices and safety protocols
