Roles & Responsibilities

• Conduct thorough psychosocial assessments to understand each patient holistically
• Develop individualized treatment plans based on evidence and client goals
• Deliver evidence-based therapeutic interventions across the lifespan
• Provide individual and family therapy to support mental and emotional well being
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues for coordinated care
• Coordinate referrals and follow-up to ensure continuity of care
• Maintain clear, timely behavioral health documentation that supports patient care and compliance
• Engage with the community and underserved populations to promote access and equity

Qualifications

• Active LISW licensure in Iowa required
• Master's degree in Social Work from an accredited program required
• Minimum of 2 years of clinical social work experience preferred
• Experience in rural or medically underserved settings is a plus
• Skilled in trauma-informed care, substance use treatment, and common behavioral health conditions such as depression and anxiety
• Strong interpersonal, organizational, and documentation skills

Education

• Master's degree in Social Work from an accredited program
